what changes have been introduced in the new proposal. Main changes in Helsinki 2008 and current proposal Considerations regarding the use of placebo The DoH 2008 contemplates restrictions on the use of placebo (Paragraph 32): “The potential benefits, risks, costs and efficacy of any new intervention should be assessed by comparison with the best existing proven intervention, except in the following circumstances: – The use of a placebo,

which are generally covered by the DoH 2008. A controversial issue is whether the “best available proven intervention” is the one that is available locally or whether international standards should be used to determine the best comparator for a given condition. In this regard, we share the general idea of defining standard care as the care that the population would receive if the health policies were working properly in that place (for review see reference)13. 13 Along these lines, it has been proposed that if the research is financed by external agents (e.g., international multicenter research), the same standards of care should be required as those that the study participants would receive if the study were carried out in the sponsoring country,13 while if the research is based on local needs, the standard of care should be in line with the reality of the host country. A policy of this type may allow the use of a comparator below international standards, provided that there is a real possibility of implementing the intervention in the local reality and that the vulnerability of the research subjects is not being exploited for the benefit of other populations, but that the results of the study can be used by the same population that has participated in it. Considerations regarding vulnerable populations The word vulnerable comes from the word “vulnerable”.

Declaration of helsinki

The verification of compliance with ethical principles in health research legitimizes its practice before society and makes it possible to resolve ethical dilemmas in the face of new research interests and methods. In Colombia, Resolution 8430 of 1993 is one of the main ethical guidelines that regulate health research. Since it has not been revised or updated since its adoption, it is necessary to assess its validity and sufficiency to address the potential ethical dilemmas that currently arise in health research in the country.in this context, some gaps and contradictions are detailed, as well as aspects that require a thorough review, based on a broad conception of the areas and methods of health research.after discussing the main shortcomings and inaccuracies, alternatives are proposed so that the Resolution responds to the current needs of the country regarding ethics in health research with human beings.
